I understand the risk of personal injury associated with participation in activities at The Beat Dance Studio. I agree to waive any and all claims of liability, and hold The Beat Dance Studio harmless if such injury does occur. If a parent/ emergency contact cannot be reached in the event of such injury, I give The Beat Dance Studio permission to provide medical attention. I fully and knowingly agree to fully assume and accept all such risks, known and unknown, dangers and hazards, associated costs, and assume full responsibility for my child’s participation. I give my consent for my child to be included in any promotional materials (videos/photos) that may be used for The Beat Dance Studio. I have read & agree to be bound by the Studio Policies (located on the studio website). This includes Payment & Cancellation policies. I have read this waiver and release agreement, fully understand its terms, and understand that I have given up substantial rights by signing it.
